After our discovery call you will fill out a questionnaire that allows me to deep dive into your health history, current habits and routines, things you may have tried, roadblocks you have encountered, and any other key information you are able to provide. This will give me a full picture of where you are at so when we do connect again we can get straight to what matters most.
